What is a SAP evaluation?

A Substance Abuse Professional (SAP) is a clinician who has completed specialized training and testing required by the U.S. Department of Transportation. When an employee in a safety-sensitive position violates DOT drug and alcohol regulations — a positive test, a refusal to test, or another violation under 49 CFR Part 40 — only a qualified SAP can perform the evaluation required before that employee can be considered for return to duty.

The SAP doesn’t advocate for the employer or the employee. The SAP’s role is to protect public safety by professionally evaluating the employee and recommending appropriate education and/or treatment, follow-up testing, and aftercare. At Mana Life Counseling, SAP services are provided by Heather Dick, LCSW — a nationally certified Master Addiction Counselor (MAC) and Certified Advanced Alcohol and Drug Counselor (CAADC) with over 14 years of clinical experience.

Who needs a SAP evaluation?

DOT regulations cover employees in safety-sensitive functions across the transportation industry, including:

  • CDL / truck drivers — operators of commercial motor vehicles over 26,001 lbs GVWR, vehicles designed for 16+ occupants, or placarded hazmat vehicles (FMCSA)
  • School bus drivers and passenger transport drivers
  • Rail workers — locomotive engineers, conductors, trainmen, switchmen, dispatchers, signalmen (FRA)
  • Aviation employees — flight crews, flight attendants, aircraft mechanics, dispatchers, air traffic control, screeners (FAA)
  • Pipeline workers — operations, maintenance, and emergency-response on pipeline or LNG facilities (PHMSA)
  • Maritime workers — mariners operating under a license, certificate of registry, or merchant mariner’s document (USCG)

The return-to-duty process, step by step

1. Initial face-to-face evaluation. A comprehensive clinical assessment including standardized testing and a biopsychosocial interview covering your history, the violation, and contributing factors. DOT rules require the SAP to recommend education and/or treatment in every case. The SAP sends the initial evaluation report to your Designated Employer Representative (DER).

2. Education and/or treatment. We can assist you in contacting providers and arranging services, and we maintain regular contact with your treatment provider to monitor progress and confirm the plan is appropriate.

3. Follow-up evaluation. A second face-to-face evaluation determines whether you’ve complied with the recommendations and measures the success of treatment. If you’ve complied, the SAP sends a follow-up report to the DER with a recommended aftercare and follow-up testing program.

4. Return-to-duty test and follow-up testing. Per DOT regulations, return-to-duty and follow-up tests are directly observed. Your employer makes the final hiring/return decision; the SAP report makes you eligible.

Who pays varies: some employers cover SAP services through an Employee Assistance Program or directly, while many employees pay on their own. Our SAP evaluation is a flat $450 — no hidden fees.

How fast can I get a SAP evaluation?

We typically offer same-week appointments. The overall timeline back to duty depends on the education or treatment recommended and how quickly you complete it — starting the evaluation promptly is the single biggest thing you control.

Will the SAP just clear me so I can go back to work?

No SAP can do that. DOT rules require a recommendation for education and/or treatment in every case, and the SAP serves public safety rather than either party. What we can promise is a fair, professional evaluation and a clear, achievable plan.

Does my employer have to take me back after I finish?

Completing the SAP process makes you eligible for return-to-duty testing, but the employment decision belongs to your employer. Your SAP records also follow you through the FMCSA Clearinghouse for future DOT employment.

What does the SAP evaluation cost, and does insurance cover it?

Our SAP evaluation is a flat $450. SAP evaluations are generally not covered by health insurance, though some employers and EAPs cover the cost. Any recommended treatment may be billable to insurance separately.

Can the evaluation be done remotely?
